End of Life Care Consultation may include:

Advanced management of distressing symptoms including pain, nausea/vomiting,
​shortness of breath, anxiety, constipation
Evaluation of emotional and spiritual needs
Collaboration with your healthcare team
Life closure activities
Preparation for the dying process
Family grief support

Consultation Fees
Initial Consultation 60-90 minutes:                       $250 
Follow up visit 30 minutes:                                     $125
Grief and Bereavement Support                            $125
Monthly package *                                                    $450
                            *for complex or intense needs requiring frequent visits/communication/prescribing
